（项目名称）招标公告1．招标条件1.1本招标项目“学府路人行天桥”项目监理 （项目名称）已由成都市温江区发展和改革局 （项目审批、核准或备案机关名称）以温发改投资[2017]138号 （批文名称及编号）批准建设，项目业主为成都九联投资有限公司 ，建设资金来自项目业主按国家有关规定筹集 （资金来源），项目出资比例为100% ，招标人为成都九联投资有限公司 。项目已具备招标条件，现对该项目进行公开招标。1.2本招标项目为四川省行政区域内的国家投资工程建设项目，项目在四川省投资项目在线审批监管平台上的项目编码为：2017-51 *** -205421,成都市温江区发展和改革局 （核准机关名称）核准(招标事项核准文号为温发改投资[2017]138号 )的招标组织形式为委托招标 。招标人选择（本招标项目在省发展改革委指定比选网站上的项目编号为PF20170904003 ）的招标代理机构是四川华通建设工程造价管理有限责任公司 。 2．项目概况与招标范围2.1工程地点：成都市温江区学府路。2.2招标范围：本项目施工直至竣工验收合格及整体移交、工程缺陷责任期全过程的监理服务。2.3建设规模及内容：新建人行天桥，主桥净宽约3.3米，天桥共设计5个梯道。2.4标段划分：本次招标为一个标段。2.5监理服务期：监理服务期暂定28个月（从本工程施工开始到缺陷责任期结束的全过程的监理工作、工程审计时的相关配合工作及提供相关服务的全部内容。缺陷责任期按国家有关规定，其中，施工监理服务期4个月，缺陷责任期监理服务24个月） 3．投标人资格要求3.1本次招标监理标段要求投标人须具备以下资格：3.1.1法人资格：具备独立法人资格（有效的营业执照）；3.1.2资质要求：具备建设行政主管部门颁发的市政公用工程监理 *** 及以上资质；3.1.3总监理工程师：拟派往本项目的总监理工程师须具备全国注册监理工程师证书（专业为市政公用工程）；3.1.4业绩要求：2014年以来具有1个工程造价500万元及以上的市政公用工程监理业绩。3.1.5财务要求：近三年（2014、2015、2016）以来均不亏损；3.1.6信誉要求：未处于财产被接管、冻结、破产状态，未处于四川省行政区域内有关行政处罚期间。3.1.7其他要求：①投标人应具有针对本标段的 《成都市建设领域市场主体单位信用记录 （评价）网络登记表》、《成都市建设领域从业人员信用记录（评价）网络登记表》。②省外企业：企业注册地不在四川省行政区域内的省外企业须提供在有效期内的《四川省省外建筑企业入川承揽业务验证登记证》或带二维码的《四川省省外施工、监理企业入川承揽业务信息录入证》；③投标人在招标公告中的发售招标文件（或投标报名）首日，投标人在成都市城乡建设委员会网站“成都市建筑市场信用信息管理系统公示平台”企业60日的信用综合评价得分排名须为-市政类监理前50（含）名。并在人员、设备、资金等方面具有相应的能力。 3.2本次招标不接受联合体投标。 3.3各投标人均可就上述标段投标，可以中标的合同数量不超过1个标段。 4．招标文件的获取4.1凡有意参加投标者，请于 *** 上午9：00时至 *** 下午17：00（北京时间，下同），使用CA数字证书在成都市公共资源电子交易云平台(www.cdggzy.com）（以下简称“电子交易云平台”）进行在线投标报名。投标人为联合体时，先由牵头人使用CA数字证书在线报名并添加联合体成员单位后，各联合体成员单位使用CA数字证书在线进行确认。获得“网络投标报名回执”，并从电子交易云平台下载招标文件等资料。招标文件下载的时限以上述约定的时限为准，逾期将不能下载。 4.2招标文件每套售价 150.00 元，售后不退。通过电子交易云平台下载招标文件的投标人应在投标截止时间之前递交投标文件的同时向招标人或招标代理机构支付购买招标文件的费用。 4.3 招标人不提供邮购招标文件服务。 5．投标文件的递交5.1 投标文件递交的截止时间(投标截止时间，下同)为 ***
